The Kentucky Council of the Blind sincerely thanks you for applying for 
the Assistive Technology Grant for 2019.KCB established this grant 
program to enrich the lives of its members by partially underwriting the 
cost of often expensive assistive technology.Your application 
demonstrates that there is a need for this type of assistance in the 
blind and visually impaired community and will serve as strong testimony 
when we reevaluate this grant program.

The Scholarship Committee had many strong applicants.After much 
consideration, the committee has approved your request for 
assistance.You are being awarded $500 in assistance for the purchase of 
(item) from (company name/website).

As stated in the guidelines for this program, you have 90 calendar days 
from November 16^th, 2019 to provide your portion of the cost of the 
item to KCB or the seller directly.Your portion for the cost of the item 
must include any applicable taxes and shipping charges.Once either KCB, 
or the seller has received your portion of the purchase cost, KCB will 
disburse its portion.

If you are going to pay the seller directly, please provide KCB with an 
account number that can be referenced for payment purposes.KCB must 
obtain an invoice from the seller before we can disburse our portion.Any 
technology grant funds that have not been disbursed by February 14^th , 
2020 will be retained by KCB.If applicable, any funds that you have 
provided to KCB directly up to that point will be returned.Once again, 
we would like to congratulate you for receiving our tech grant award and 
we look forward to hearing from you.
