[Scholarship-comm] AT Grant Application updates

Everyone:


Attached is the latest tech grant application.  I corrected my spelling 
mistakes and deleted the words that we discussed as well as added the 
word annually after $500.

-Modified the following  sections:

Section V. Application:

4. Written verification of legal blindness from an ophthalmologist, 
rehabilitation counselor, or other reasonable authority (obtained within 
the past year), including a description of the applicant’s eye 
condition, visual acuity, and field of vision.  *Or a signed Visual 
Verification form to be completed by a competent professional authority. 
This form is included with this application.*This requirement may be 
waived upon request at the discretion of the Scholarship Committee.


B. The Scholarship Committee Chair shall *recommend to the KCB Board of 
Directors of its nominee(s) to receive the Assistive Technology Grant* 
including the name of the grant recipient and the item to be purchased 
and any other relevant information for their *final* approval.

D. The grant recipient(s) are required to provide their portion of the 
matching funds with in ninety (90) days from the date of the award. Any 
grant funds that are not dispersed with in the ninety (90) days, shall 
be retained by KCB.*


Additionally, the second to last page of the word document is a release 
form.  I propose that we delete this page simply because we already have 
a visual verification form included with the application or the 
applicant can provide written verification from a professional.  Lastly, 
I have e-mailed the board about our discussion of the under $500 50/50 
match.  Once we have a consensus, I will modify the application as needed.
