I'll save attaching the document this time; I'll just send it to you off 
list if you want Patty to review for large print formatting.  If anyone 
wants it, I'll send it along.


Corrections:

Corrected "with in" to "within".

Corrected "waved" to waived".

Removed commas prior to etc...

Removed hyphen in the word standalone.  I don't see where they are 
missing i.e. five-hundred for example.

Inserted lines to provide space for yes/no questions and grant amount.

Corrected "Or visual verification" to "acuity, or visual verification".

Changed "legally blind" to "visually impaired" and "legal blindness" to 
"visual impairment".


As for the webpage, if your going to host a copy of the application 
there, that sounds great to me and I can link to it in the 
announcement.  I would like to get all of the editing done before 
Monday's meeting if possible.
